After Care Services & Transition Planning

Emancipation and Aging Out
 

Transitioning Planning & Aftercare Program: The planning process begins at age 15 and you will have planning sessions every six months until you exit care (either by guardianship, adoption or aging out at 21). CFSA provides support and resources to youth who age out of care at age 21 for two years (until your twenty-third birthday). You will be assigned a case worker through one of the Healthy Families/Thriving Communities Collaboratives (see contact information below).

Foster Club Transition Toolkit: CFSA uses the Foster Club Transition Toolkit to guide you through the transition process.

Post- Care Housing Resources:
Wayne Place Housing Program: Housing program located in Southeast DC from youth aging out of care. Program is run by the Far Southeast Family Strengthening Collaborative (see contact information below)
Genesis Housing Program: This is a  multi-generational housing program located in Northwest DC for youth who age out of care and have children.
Rapid Housing Assistance: You can receive assistance paying your rent if you meet certain qualifications.

Your Social Worker-your social worker’s email address is usually their firstname.lastnams@dc.gov if they are from CFSA but it might be different if your case is managed by an outside agency (Lutheran Services, Boys Town, etc.).   2. Your Guardian Ad Litem (GAL): All foster youth are assigned a GAL through the Children’s Law Center or the DC Superior Court Child Abuse and Neglect Attorney Practitioners (DC CCAN Panel)—see contact information below.

DC Superior Court Child Abuse and Neglect Attorney Practitioners (DC CCAN)
About: Attorneys from the DC CCAN pan serve as Guardian ad Litems for youth in foster care. They advocate on behalf of the best interest of the child. They will help with an array of issues faced by youth in foster care such as placement or education. They can also be appointed “expressed interest” (the attorney will work on behalf of youths’ interests) attorney by a family court judge.
